To give you the bottom line first, under-eye fat repositioning does improve them to some degree. Honestly, no one has zero pigment under their eyes. It's just a matter of more or less.

There are two reasons under-eye fat makes dark circles look worse. One is that the tiny blood vessels inside the fat can show through, and the other is that the height difference between the puffy fat and the tear trough creates shadows and shading, which makes the area look a bit darker. Those are the two reasons.

Under-eye fat repositioning is the surgery that addresses both of them, so you could say it improves things to the extent that the shadows were making them look worse.
